Requirements
All
applicants must submit the following requirements to the PSHS
System Office of Admissions in Diliman, the
nearest Regional PSHS campuses, DOST
Regional Offices or Provincial
Science & Techonology Centers.

The
fully accomplished application form with four (4) 1"
X 1" identical recent photos

Parents'
statement/affidavit that the child has no pending
application for immigration status in a foreign country

A
PhP75.00 non-refundable processing fee.

A
certificate of good moral character from the Head of
School or Principal

For
those who did not make it to the upper 10% of the class, a
letter of recommendation as to their having special
aptitude in Science and Mathematics from their Guidance
Counselor or Science and Mathematics teacher or school
principal, and supporting documents such as but not
limited to report cards, aptitude test results, and merit
certificates.

(2) self-stamped long window envelopes.
